Mobile Primary Care

Grace and Hope provides quality primary healthcare to the home-bound and home-limited and offers the most convenient, detailed, and compassionate medical care to you or your loved one. The only difference is that instead of traveling to your doctor office the nurse practitioner comes to your home and building.

We see patients with multiple and complex health issues in private homes, assisted living facilities, and group homes.

We work with patients and their families to provide primary care, prescription management and refills, diagnose illness, conduct physical exams, perform wellness check, perform blood work, monitor Coumadin levels , prescribe in home services like physical therapy , and collaborate with your health care teams already in place. We offer referrals for x-rays and EKG’s in the home.

Mental health provider/ Behavioral Health Services

Mental health providers diagnose mental health conditions and provide treatment. Grace and Hope provides psychiatric and mental health services to individuals and families in a therapeutic and non-judgmental manner. We listens to patients with empathy and passion. Here at Grace and Hope , we assess, evaluate and re-evaluate, perform diagnostic evaluation, diagnose, treat, assist patients to acquire, develop and, maintain therapeutic skills they need for self-empowerment.

We respect the uniqueness of each patient and this allows them to be a part of their own treatment plan. Our goal is to help our patients to achieve a level of optimum stability and remission from symptoms of mental health problems.
